Below is a link to the necessary intake forms that must be completed prior to the initial appointment at Live Well Services, Inc. Mandatory Intake Packet (PDF)Please complete 1 form for your insurance company and 1 form for your primary care physician. Additional copies may be completed at your discretion for any additional friends or family members whom may need to access your medical information or speak with our staff on your behalf. We strongly urge young clients 14 and up to sign a release on their parents behalf.
