How do I apply for services?
Individuals may apply for services by contacting our office by telephone (215-968-7600) during daytime working hours. If no one is available to answer your call, please leave a message, and one of our office assistants will call you back promptly.
How does it work?
An intake worker will help to determine if our services can meet the individual's needs. Eligible persons are accepted as clients of Live Well Services, Inc. without regard to race, color, sex, age, religion, or disability.
What insurances are accepted?
Not accepting insurance at this time.

When do I arrive for my first outpatient Intake appointment?
Expect to arrive 15 minutes early for your first appointment to complete the paperwork that is needed prior to you or your child seeing an intake specialist. The intake person will conduct a thorough interview and discuss all your treatment options. Also, please be sure to bring a copy of your health insurance card; it is an important part of our records.
What do I do if I need to cancel an appointment?
It is necessary to call at least 24 hours in advance to cancel your appointment by contacting our office by telephone (215-968-7600) during daytime working hours. A $35.00 service fee will apply to all appointments that are no-shows or are cancelled with less than 24 hours notice.
